Complete Your Intake Forms

Download and complete your new patient intake forms before your appointment. Bringing completed forms saves time and allows your physician to review your history before you arrive.

Attend Your Consultation

Arrive 10 minutes early with your completed forms, insurance card, and photo ID. Your physician will conduct a thorough evaluation and discuss your options in detail.

ZMD ENT accepts a wide range of insurance plans for covered ENT services. We are committed to making excellent care accessible and will work with you to maximize your benefits and minimize out-of-pocket costs.

Please note that coverage varies by plan and procedure. Some services, including cosmetic procedures, hair transplantation, and certain aesthetic treatments, are not covered by insurance and are provided on a self-pay basis. We will always discuss expected costs before any treatment begins.

Transparent Financial Policy

We believe in complete transparency about costs. Here is everything you need to know about payment, insurance, and financing at ZMD ENT.

Insurance & Billing

We will file claims with your insurance on your behalf for all covered services. You are responsible for any co-pays, deductibles, and non-covered services at the time of your visit.

Please bring your insurance card and a valid photo ID to every appointment. We will verify your benefits prior to your visit and notify you of any expected out-of-pocket costs.

Self-Pay & Cosmetic Services

Cosmetic procedures, hair transplantation, and certain aesthetic services are not covered by insurance and are provided on a self-pay basis. Pricing is provided during your consultation.

We accept all major credit cards, cash, and offer financing through CareCredit for qualified patients. A deposit may be required to schedule surgical procedures.

Cancellation Policy

We ask that you provide at least 24 hours' notice if you need to cancel or reschedule your appointment. This allows us to offer your appointment time to another patient who may be waiting for care.

Late cancellations (less than 24 hours) and no-shows may be subject to a cancellation fee. Surgical cancellations may result in forfeiture of the surgical deposit depending on timing.

Financing Options

We understand that elective procedures represent a significant investment. We offer financing through CareCredit to help make treatment accessible. Approval is subject to credit qualification.

Our secure patient portal gives you 24/7 access to your health information, appointment details, and communication with our team — all from your phone, tablet, or computer.

You will receive an invitation to create your patient portal account after your first appointment. If you have not received an invitation, please contact our office and we will send one promptly.

New patient consultations typically take 45–60 minutes, including check-in, the physician evaluation, and time for questions and discussion. More complex cases or those requiring in-office testing may take longer. We schedule adequate time for each new patient to ensure a thorough, unhurried experience.
Yes. You have the right to access your medical records. You can request records through our patient portal, by calling our office, or by submitting a written records release request. Records are typically available within 5–10 business days. A nominal fee may apply for extensive records requests per California law.

If your care requires coordination with another specialist — such as a neurologist, oncologist, or allergist — we will provide a referral and coordinate communication to ensure seamless, comprehensive care.
